Carezhen provides software that helps healthcare organizations manage appointments, patient communications, and care coordination. This Privacy Policy describes how information is collected and used when patients interact with the Carezhen platform, including the public appointment booking experience.
SMS Consent and Communications
Patients opt in by completing the online appointment booking form on the Carezhen website. During the booking process, the SMS consent checkbox is unchecked by default, and patients must actively check the box to provide explicit consent before they can submit their appointment request. The consent statement clearly explains the types of SMS messages they may receive, including appointment confirmations, appointment reminders, appointment rescheduling notifications, patient support messages, post-visit satisfaction surveys, review requests, and preventive care reminders. It also informs patients that message frequency may vary, message and data rates may apply, and that they can reply STOP to opt out or HELP for assistance. Information We Collect
We collect the information you provide when booking an appointment, such as your name, date of birth, phone number, email address, and any notes you share with your care team. We also collect basic technical information (like device and log data) needed to operate the service securely.
